Some changes in 5.2 that were made for dynamic remastering caused a problem in RMS. A blocking AST would not get triggered resulting in a locking user to not dequeue the lock. And you would then end up with the chain reaction you describe. I got the patch as CSCPAT_0158011 from Colorado.
